Thank you sooooo MUCH! This is such a blessing to me! I filled it out and it is valid for 9 pills for any first time user. My Dr has given me samples and I love this stuff! I have no insurance and the cost of a prescription for this stuff is about $200. Again thanks so much for posting this!
